Family Educational Rights and Privacy Act


The School of Dentistry adheres to the provisions of the "Family Educational Rights and Privacy Act" (FERPA) in all matters relating to the confidentiality of student records. Information about students or former students will not be released without the consent of the student other than for those exceptions stated in the Federal Act.

University General Counsel has determined that faculty members writing letters of recommendation for students seeking positions in programs of post-doctoral study need to secure the permission of the student prior to disclosing information about academic performance in that letter. This includes such data as class rank, National Board scores, and GPA.

In order to expedite this process, a copy of the waiver or release form (PDF format here) is posted on this site. Print a copy for each person who is being requested to release academic information and complete the top portion of the form. Present this form with your section completed to the person you have requested to make a recommendation. They will then complete their portion of the document and present it to the Office of the Associate Dean for Academic Affairs. At this time, information in your academic record will be released to the person you have designated. The form will be placed in your student file.
